The University of Miami Health System, "UHealth", Department of Pediatrics has an exciting opportunity for a Case Worker 1 to work at the Miiler School of Medicine.

The Case Worker assists patients in keeping appointments, identifies barriers to care, coordinates the development of a treatment plan, and monitors the implementation and follow-up of treatment/service provided or ordered by health care providers.

  • Conducts patient evaluations to obtain information on the patient's medical and mental health, treatment and recovery to develop an effective treatment plan.

  • Ensures patients are enrolled in insurance and free or low-cost social service programs for which they are eligible (e.g., food stamps, WIC, housing, SSI, Medicaid/Medically Needy Program, Ryan White, and ADAP, etc.).

  • Learns the requirements of the new Affordable Care Act and the · enrollment process to ensure patients know how to appropriately enroll or transition their care from Ryan White to a health care exchange or other insurance program that will allow them to maintain their care with our program.

  • Identifies and resolve real or perceived barriers to care, (e.g., disclosure, cultural beliefs, denial, transportation, childcare, support systems, functionality, insurance, etc.) to enable patients to adhere to care recommendations and improve their health outcomes.

  • Assists with patient-provider communication.

  • Provides medication adherence education, assessment, and counseling.

  • Conducts secondary prevention and lifestyle management interventions.

  • Provides clients with information on available clinical trial opportunities and facilitates linkages to research staff.

  • Educates and counsels patients around issues related to their illness and/or life situation and assists in finding additional information from websites and agencies.

  • Maintains knowledge of community health and welfare resources and the variety of programs offered.

  • Maintains confidentiality of all records, data, and information as required by HIPAA regulations and other regulatory agencies

  • Documents filing and actions taken, according to departmental guidelines and standards.

  • Assists with record audits for documentation and record compliance.

  • Adheres to University and unit-level policies and procedures and safeguards University assets.

Department Specific Functions:

  • Travels to homes, daycares and other natural environment locations as necessary to conduct the Individual Family Support Plan meetings.

  • Participates as a member of an interdisciplinary team while serving as the single point of contact for families in need of early intervention services.

  • Contributes to the development of a family support plan and monitors its implementation and modification.

  • Participates and contributes to the development of the Individual Family Support Plan (IFSP) and monitors its implementation by assisting parents of eligible children gain access to early intervention services as well as additional support services available in the community.

  • Assists parents of eligible children in gaining access to early intervention services as well as additional support services available in the community.

  • Provides education to families of eligible children on the services available in the community to enhance child developmental activities.

  • Collaborates with personnel in community-based agencies to develop appropriate intervention strategies for eligible children and their children.

  • Maintains a resource and referral file to assist families in finding timely and appropriate community-based programs and resources.

  • Maximizes the continuity of care provided to enhance developmental potential.

  • Coordinates activities with other case-related medical and non-medical personnel.

  • Facilitate authorization of appropriate level of services for each child and family enrolled in the program.

  • Documents services provided in the prescribed manner and completes paperwork prescribed by the Department to include documents, such as the Individual Family Support Plan (IFSP) requested by the state, etc.

  • Completes required paperwork as needed for input into data and billing systems.

CORE QUALIFICATIONS:

  • Bachelor's Degree required from an accredited university with an emphasis in the areas of psychology, social work, health education, interdisciplinary sociology, early childhood, child development or special education

  • Minimum 1 year of relevant experience required with young children and families in a teaching, case management or counseling role, or with individuals with special needs and/or developmental delays of any age in a case management.

  • Bilingual in English and Spanish preferred.

  • Skill in completing assignments accurately and with attention to detail.

  • Ability to analyze, organize and prioritize work under pressure while meeting deadlines.

  • Ability to process and handle confidential information with discretion.

  • Ability to work evenings, nights, and weekends as necessary.
