Description:The Community Health Worker (CHW) is a frontline public health worker who is a trusted member of and/or has a remarkable understanding of the community served. This trusting relationship with the community enables the CHW to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.
Responsibilities:A CHW builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy. The community health worker is also adept at improving health behaviors and increasing health awareness and health literacy through evidence-based and evidence-informed programs. The CHW applies behavior change strategies and motivational interviewing techniques to motivate participants to develop and maintain the skills needed to adopt a healthy lifestyle to prevent or manage chronic conditions.
The CHW will support the department in fulfilling the following community health goals: decrease avoidable hospital utilization; improve overall health through strategic partnerships, particularly with those addressing social determinants of health, and strengthen integration and linkages by wrapping community health programs and services around defined populations, services of the Network and both hospitals.
Supports the Mission of Trinity Health and Holy Cross Health.
What you will need:
- Comprehensive knowledge of the local community based on personal lived experience and the ability to articulate the lived experience and perspective.
- High school graduate or GED required.
- Certified Community Health Worker or ability to earn a CHW certificate, preferred.
- One to three years of community involvement experience (i.e., community health worker, volunteer for community-based organization, outreach worker, community navigator, etc.)
- Work requires ability to communicate orally and to hear or communicate with focus population.
- Ability to inspire and motive others.
- Must be comfortable working and serving in a diverse and inclusive environment, and operating in a collaborative, shared leadership environment with clinical supervision where applicable.
- Bi-lingual skills to work with patients in their native language are preferred, certification as a medical interpreter/translator is a plus.
- Basic Life Support (BLS) certification, preferred.
- Ability to work flexible hours, including evenings and weekends.
- Proficient computer literacy (email, internet, Microsoft Word and PowerPoint), preferred
Pay Range: $22.83-$31.96Pay is based on experience, skills and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
