Description: Join our team in Eugene Oregon Lane County as an Advanced Practice Clinician (APC) - Nurse Practitioner!
We are looking for a dedicated provider to join our integrated care delivery program at Senior Living Facilities and homes. This contract position (4-7 months) focuses on our highest-risk health plan members, offering a fulfilling opportunity to provide comprehensive care and conduct advanced care planning conversations.

Paid up to $80-85 per hour W2
Our Institutional Special Needs Plan (ISNP) is a type of Medicare Advantage plan tailored to individuals requiring care in a long-term facility for at least 90 days. ISNPs are designed to meet the medical, social, and emotional needs of individuals living in or receiving nursing care at home, combining the benefits of Original Medicare with prescription drug coverage.
Key Responsibilities:
- Conduct comprehensive assessments
- Manage acute and chronic conditions
- Collaborate with the care team
- Provide counseling, education, and advanced illness planning
Preferred Qualifications:
- 1+ year of clinical experience, preferably in long-term care
- Proficiency in geriatrics and chronic illness management
- Strong computer skills for medical documentation
