Schedule: optional part time position picking up hours Monday- Friday 0800-1630
Job Summary:
Responsible for facilitating the care of designated patients and their families during a patient's hospital admission or outpatient visit. Coordinates discharge plan from pre-admission through discharge follow up. Provides psychosocial assessment and updates that address the patient's evolving needs. Establishes a plan of care that provides maximum benefit to patient and their families. Other duties as assigned.
EXPERIENCE DESCRIPTION:
Minimum of two years experience in an acute care setting preferred.
EDUCATION DESCRIPTION:
Masters Degree in Social Work with casework in medical social work.
SPECIAL SKILLS DESCRIPTION:Interpersonal skills necessary to obtain information from patients, to provide counseling to patients/families, and interact effectively with patients, families and hospital personnel. Analytical skills necessary to assess patients needs, develop associated discharge planning and develop solutions to problems concerning patients mental and physical well being.
LICENSURE DESCRIPTION:Current State of Wisconsin license as social worker required
